Synergy Wellness

Services include:

Mental health needs assessment (NASC)
Service coordination to access funded community and disability supports
Support for individuals and whānau to plan and review care and support arrangements

Synergy Wellness accepts referrals from specialist mental health services, as well as GPs, other health or social service providers, and through self‑referral, where eligibility criteria are met.

Referrals require supporting clinical information and are intended for people who experience ongoing disability due to a significant mental health condition. Eligibility criteria apply.
